Facility: Helen Porter Rehabilitation & Nursing
Need 11/10 start - Assignment for 16-32 weeks

CNA for LTC: Floating required. A minimum of every other weekend and/or schedule adjusted to meet the needs of the unit and patient care. Cat on unit - cannot be allergic. Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review. Travelers must work 2 out of the 4 holidays: Thanksgiving, Christmas, Christmas Eve, and New Years Day. Requirements: • Prior travel required • 2 years of experience required • Long term care experience preferred • Questionnaire attached to position files must be filled out and added to sub packet • Epic experience from within the past 2 years is required and nonnegotiable Additional Information: • IVS to OFFER • Monday to Sunday • No block schedules for any reason • Bi-weekly orientations on Monday, next available is 11/24 • Please ensure all employment gaps over 90 days are explained • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit Disclaimers: • Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended • Vehicles should be equipped with all season or snow tires during the months of November – April
